You may download Acrobat Reader by clicking the graphic at the bottom of the list. These forms
should be turned in to the Director on Duty (look for the blue shirt), or at the information
station.
Referee Thumbs Up Form
Does the referee who worked your game deserve a complement or vote of confidence? Use this form to
let them know they did a great job, or handled a tough situation well.
Referee Complaint Form
Just like our players, sometimes our referees need a little coaching. Our Zero Tolerance Policy on
Referee Abuse requires that you use this method to provide feedback to our referees. Remember, many of
our referees are kids themselves. Be constructive in your feedback.
